Insurance
Insurance cover is provided as part of player registration fees.
Insurance claims must be completed and submitted within 30 days of the incident. Please do not wait for medical accounts as these can be submitted later. Please provide information to the club via playfootball@redbacksfc.com.au as soon as possible after the incident.
You will need to complete all sections of the form, including obtaining declarations from the club and your treating medical practitioner. Please contact the club via playfootball@redbacksfc.com.au to complete the club declaration.
